Quick recap

Jamie wakes up wounded on the field at Culloden and survives. He spends years in hiding, turns himself in and serves time at Ardsmuir, where his friendship with Lord John Grey begins. Later he works at Helwater and fathers a son, William, whom he has to leave behind. In Boston, Claire raises Brianna, becomes a doctor and is widowed by Frank. With Roger's help, she finds Jamie as the printer A. Malcolm in Edinburgh and goes back to him in 1766. The reunion comes with smuggling, a second wife named Laoghaire and young Ian's kidnapping by a mysterious ship. The search takes them to the Caribbean, where Claire kills Geillis in a cave. The book ends with a shipwreck and landfall on the coast of Georgia.
