Reacher gets off a bus in an unnamed city after spotting a mugger trying to rob an elderly man. He foils the mugging and meets Aaron Shevick, who carries a bundle of cash for a loan shark tied to Dino's Albanian mob. Unknown to both, the loan shark is already dead, and Gregory's Ukrainian mob has taken over his business. Reacher poses as Shevick to meet the Ukrainians and kills two by staging a car accident. At the couple's home he learns why they sold nearly everything: their daughter has late-stage cancer and they can't afford treatment.
