Mia arrives at the childhood cabin with half-brother David, his girlfriend Natalie, and friends Eric and Olivia. The plan is to stay locked in until she gets through withdrawal. She has tried before and overdosed, dying for a moment. So when she starts seeing and hearing strange things, everyone assumes it's withdrawal.

Eric reads aloud from the Naturom Demonto, found in the cellar, and an ancient evil wakes. Mia tries to flee by car, crashes into a swamp and is caught by vines. The demon possesses her, and she becomes the ritual's first body.

Possessed, Mia scalds herself in the shower, hurts her friends and is locked in the cellar. The book explains the demon needs five souls to rise as the Abomination, and that Mia can only be stopped by fire, burial or dismemberment. The house fills with blood: Olivia, Natalie and Eric die, possessed or not.

David decides to bury Mia alive and, when he hears her heart stop, digs her up and revives her with a homemade defibrillator. The demon, driven out of her, possesses Eric. David sacrifices himself by blowing up a gas can.

With the fifth soul, the Abomination rises under a rain of blood. Mia cuts off the monster's legs with a chainsaw, but a Jeep crushes her arm. Free, her hand torn off, she splits the creature in half with the saw and watches it sink into the ground.

The film, produced by Sam Raimi, Bruce Campbell and Rob Tapert, has Ash only in a post-credits scene. Fede Álvarez said it continues the original and that fate is part of the book's evil.

In 2013, Bruce Campbell and Fede Álvarez said they ultimately wanted to merge Ash's and Mia's stories. In March 2023, Raimi repeated his interest in bringing them together. Nothing has materialized so far.
