The Inhumans live in hiding in Attilan, a city on the Moon. Their ruler is King Black Bolt, whose voice is so destructive that a whisper can level buildings, so he never speaks. Beside him is Queen Medusa, who controls her hair like extra arms. The royal family also includes Karnak, who can see the best way out of any situation, Gorgon, who causes quakes by stomping his hooves, young Crystal, who controls the elements, and Lockjaw, a giant teleporting dog. The king's brother, Maximus, went through the Terrigen Mist and came out with no powers at all. He believes Attilan is failing and that the Inhumans should return to Earth.
Maximus stages a coup. His soldiers attack the royal family and shave Medusa's head to take away her power. Crystal sends Lockjaw to carry the king, the queen, and the others to Earth, and they land scattered across Hawaii. Crystal is stuck in Attilan and only escapes later.
On Earth, each of them gets by however they can. Black Bolt is arrested by Honolulu police for causing a disturbance. Medusa, powerless, searches for her husband and gets help from Louise, a human scientist obsessed with the Moon. Karnak hits his head, loses faith in his gift, and ends up on a marijuana farm, where he gets involved with Jen. Gorgon befriends some local surfers. Maximus sends Auran, head of the guard, to hunt them all and teams up with scientist Evan Declan, who wants to study Inhuman DNA. Gorgon sacrifices himself to save the others when an underground hideout collapses.
The family reunites and learns that Black Bolt saw the coup coming and had a plan ready. They return to Attilan to negotiate, and Black Bolt agrees to let his brother undergo a new Terrigenesis in exchange for the throne. Maximus breaks the deal. Karnak convinces Auran to help and tries to revive Gorgon with the Mist. Gorgon comes back with a feral mind, kills Declan, and runs off. He ends up locked up with Karnak, who helps him remember a little of the past.
In the end, Maximus reveals that the city depends on him: a failsafe needs his fingerprints at regular intervals, or Attilan falls. He also confesses that he fabricated the evidence that led to their parents' deaths. Black Bolt locks his brother in a bunker and evacuates the people to Earth with help from Eldrac, an Inhuman who works as a living portal and dies along with the city. Attilan is destroyed. Maximus is left trapped in the ruins, and the Inhumans start over as refugees on Earth.
