Johnny Blaze is hiding out in Eastern Europe, trying to keep the Ghost Rider spirit locked inside him. The curse is eating him alive and he is barely holding on. The story starts at a monastery, where Moreau, a hard-drinking, foul-mouthed monk, warns that a man named Roarke is sending men to grab a boy called Danny. The attack comes, and Danny escapes with his mother, Nadya.
Moreau tracks down Johnny with an offer: if the Rider finds and protects the boy, the Church will lift the curse and give him his soul back. Johnny agrees. Roarke, who is actually the devil in human form, wants Danny for a ritual to move into a new body, since his current one is wearing out. Danny is his son: years ago, Nadya made a deal with Roarke to save her own life.
Ray Carrigan, Nadya's ex and a mercenary working for Roarke, captures mother and son. The Rider shows up and wrecks the convoy, but Danny gets taken again, and Roarke casts a spell that hides the boy from Johnny. Johnny wakes up in a hospital, convinces Nadya to trust him and they follow the leads. The Rider levels Carrigan's hideout with a giant excavator wrapped in hellfire and leaves him near death. Roarke brings him back as Blackout, a creature that rots anything he touches.
They rescue Danny and take him to a hidden monastery. There, Moreau reveals the spirit's origin: the Rider is Zarathos, an angel of justice driven mad after being dragged into hell. Johnny admits he made his deal out of selfishness, because he couldn't accept his father's death. Moreau exorcises the spirit and Johnny is just a man again. But the monks have their own plan: kill Danny. Before they can, Carrigan storms in and takes the boy.
Powerless, Johnny, Nadya and Moreau head to the ritual at ancient ruins. Carrigan kills Moreau. Danny, who has gifts of his own, gives the fire back to Johnny, and the Rider can now even ride in daylight. He kills Carrigan, chases Roarke down and drags him back to hell with his chain. Danny dies in the fight, but Zarathos regains his sanity as a spirit of justice, the flames turn blue and Johnny brings the boy back to life. In the end, he rides on, finally at peace with his curse.
